Businesses must complete the Worker’s Comp Affidavit

Homeowners may complete the Affidavit of Exemption OR must have the contractor complete the Worker’s Comp Affidavit.
Businesses must get a stamped copy returned from the Worker’s Comp. Board before submitting it with the Permit Application. Either form must be notarized.
